- New Patient Information Package
- Consent of Obtaining Medical Records Form
- Financial Policies Form
- HIPPA Regulations Acknowledgement Form
- Notice of Privacy Practices Form
You must also bring the following:
- Your insurance card
- Photo ID (License)
- A list of current prescriptions and/or over-the-counter medications you are taking, including dose and frequency
- Pertinent information about your medical and surgical history
- Any recent x-rays, MRI, CT scans, films, CDs or appropriate records you may have
