1.  How do I get to your office?

Directions are as follows:   Take 17th St. (Maricamp Road) to the traffic light on SE 18th Avenue.  There is a Regions Bank Building on the corner of SE 17th St. and SE 18th Avenue.  Turn South on SE 18th Ave.  Turn right on SE 18th St. and make quick left into 18th St. Professional Park.  Jose A. Gaudier and Lacy Jernigan, ARNP office, Suite 1202 is located on the first building on the left.

3.  How can I make an appointment?

You may contact us by phone, please be prepared to provide the following information:

  • Patient’s name
  • Address
  • Phone number
  • Birth date
  • Type of insurance
  • Reason for appointment
  • Referring physician, if applicable.

5.  What will be the cost of my visit?

Questions regarding your coverage and benefits should be directed to your employer or insurance company.  Insurance coverage varies depending on the insurance company.  The length and complexity of your medical evaluation may also determine the cost.

We are Medicare providers and accept most major insurance plans.  We provide insurance billing; anything not covered by insurance will be your responsibility.  If you have any questions regarding which insurance plans we accept or any patient billing concerns, please call us at 352-732-8630.

Payment of all copayments, coinsurances, and past due bills are due at the time of service.  If you don’t have insurance, payment is required at the time of service.  We accept cash, checks and all major credit cards.

7.    What if I miss or cancel my appointments?

It is the policy of Jose A. Gaudier, MD, PA to confirm appointments at least 24 hours prior to the date of the appointment. If a patient does not show up for a scheduled appointment and does not give at least 24 hours notice that account may be billed a charge of $50.00. Three missed appointments without proper notification may result in being discharged from the practice.

8.   What if I have problems before my appointment?

If you are having a medical emergency, you should call 911.  Otherwise, you should notify us and your other physicians for any changes of concern.  For any problems that needs prompt attention we will be glad to schedule an earlier evaluation as soon as possible.

9.  How do I get a prescription refill?

We prefer to refill your medications at the time of your office visit.  Please bring your medication list with you to each appointment.  Due to the high volume of refills request that we receive daily, it make take us up to 48 hours to call, fax or electronically send your prescription.

For faster prescription refill service we suggest to call the pharmacy of your preference and ask them to contact us with your prescription request.

10. What if I like a second opinion?

Dr. Gaudier is very supportive of their patients getting a second   opinion. He understand the patient’s goal is to get the best objective opinion and     best treatment options available.  Don’t hesitate to ask for our help, we will gladly   assist you in helping you with providing      copies of your records, setting up    the     appointment with the doctor of your choice, or any other need you may have.
